Pre Participation Medical Clearance Requirements for Varsity Student Athletes 

First Year Student Athletes (Freshman and Transfer)

Pre Participation Physical Examination:

All first year student athletes at Bridgewater State College (incoming freshman, transfer or those that have never played on a BSC team before) must have a physical examination on file with the athletic training/sports medicine department prior to the teams first practice. The Athletic Training Service Department Pre Participation Physical Exam form assures that the necessary information for medical clearance from the examining practitioner will be provided. If this form is not used there must be a written statement by the examining practitioner regarding the participation status of the student athlete on the physical submitted. The Physical Examination must be within 3 months of entry into the academic program at Bridgewater State College.

Pre Participation Athletic Health Questionnaire Form:

All student athletes must complete a First Year Athlete Pre Participation Athletic Health Questionnaire Form prior to the start of the teams first practice. This form includes completion of a health history questionnaire, student athlete demographic and contact information, along with a HIPAA disclosure, consent to treat and assumption of risk declarations. A copy of the front and back of the student athletes health insurance card must be submitted with the Pre Participation Form. The NCAA requires the verification of personal health insurance coverage.

Returning Student Athletes:
